Industrial Registry updates affidavit form

The Industrial Registry has introduced an updated Affidavit form (Form 20) and accompanying fact sheet to help parties prepare and file affidavits.

The form is provided in both PDF and Word, with an accompanying Affidavit Continuation page and Certificate of Exhibit to an Affidavit (Form 21).

The Registry urges all practitioners and parties to ensure they are using the current version of the form for future filings.
