Errors corrected on Notice of child abuse Word forms

The Word and RTF versions of the Federal Circuit Court and Family Court ‘Notice of child abuse, family violence or risk’ forms have been updated.

This follows the identification of two errors in the Word and RTF versions, however both PDF versions (plain and interactive) are not affected.

Question 21 was transposed incorrectly and should have read ‘Do you allege that drug, alcohol or substance misuse by you, or the other parent or a party to the proceedings, has caused harm or poses a risk of harm to the child?’

At question 35, the right hand column for option 2 and 3 should have read ‘What action (if any) was taken and by whom?’.
