QLS publishes new forms 4 & 5

Queensland Law Society has recently published the following updated forms:

• Form 4 – Law Practice Declaration and Trust Money Statement (Part A & Part B) – Version 13
• Form 5 – External Examiners Report – Version 13

The updated forms can be found on the Society’s website.

The changes include:

• The forms have been streamlined for ease of use;
• Form 5 has been made more comprehensive to provide more useful information;
• Completion instructions are included on the last page of the forms;
• Form 4 Part A is to be completed by all law practices that have provided legal services in Queensland;
• Form 4 Part B is to be completed by all law practices that have received trust money and/or operated a trust account;
• Form 5 is to be completed by the law practice’s External Examiner.

Due dates for the forms are:

• Form 4 Part A only – 30 April 2025
• Form 4 Part A & Part B – to be provided to the External Examiner prior to commencement of their examination
• Form 5 (together with Form 4) – 30 May 2025
